Questions about how rapid fire works

Okay, so let me start by saying that acidmods is a great community.  I think its great that everything is pretty much open source when it comes to projects.  But, I have some questions about programming RF Code it C.  Which is it: 

1. When you have a pin check for input, compare that to a 1, if yes, you switch it so it outputs a 1, then back to an input, wait for so long, then check for an input again, etc

2.  Or is it you have a pin check for input, compare that to a 1, if yes, you switch it so it sinks the pin to a ground by outputting a 0, continue this for so long, have the pin stop grounding itself by outputting a 1 for maybe 5ms, then checking for input, etc

So which method is it and why?

Also, does the matrix or cg/cg2 naturally read high or low?
