get rid of the 12V fan mod.

with the right heatsinks and original x-clamps (with artic silver 5 paste) your console should run perfectly fine with stock fans.

there is no 12v mod hell there was no signs of previous work done
this had original cheap 1st gen heatsinks so threw on 2nd gen
noise stays low but after a awhile fans kick on hard even just on idle

Your overheating, or at least it thinks it is. you may want to check the application of thermal paste.  The fans kick up in response to a thermal sensor.

i got this on my one a repair from microsoft ive just left it on my wardrobe as it got way to noisy if a fix comes up ill like to know as well

.. meh.. could be several things. i would check the thermal paste like matt said. if that doesnt work i would reflow it... maybe there is a cold solder joint and its triggering it?

You added the xclamp fix?

You probably do not have enough pressure on the chips.  The reason the fans are so loud is because the xbox is overheating and its trying to keep itself cool, so It kicks the fans voltage up to a point where it won't overheat anymore.  I would check that xclamp fix. I wouldn't necessarily tighten the screws, because if you do that you might crack the mobo. Look at your washers and see if it's on tight enough.

I have had this issue before, even though the reflow was successful, i have had it where i had to reflow it again due to the solder connection for the thermal sencore wasn't making a good contract also , make sure that the heat sync is even all the way around , one washer could be holding it up and causing the issue with the fans.. you can always text or call me if needed...

hmmm now its giving me a 2 red led rrod lol
well heres what i did i did a reflow with no clean lux
 added artic silver 5 (used a rice grain amount on each of the 3 silver parts on the cpu/gpu
its not a thick coat nor a thin one i think its in the middle
added 2nd gen heat sinks to it
added the xclamp fix used a cheesehead screw 2 washer under the board and 2 washer on top
thing was noisy as 
it kept freezing and after a while whole console would lock up
so i reflowed it again using no clean flux
worked
redid what i did as far as as5 and xclamp etc
now im getting
2 red led rrod
pos zypher board

guess it was the xclamp fix looks like it didnt like that extra washer under the board done 4 washers numerous times 2 on top and 2 on the bottom used 2 washers on top and 1 on the bottom with minimal force
finally got it fixed even the fans went back to normal
played like an hr of battlefield 3 no rrod minimal fan speed thanks guys for all the suggestions and help
believe it or not this is my first zypher board never encountered all these problems all at once seemed like everytime i fixed an error something else went wrong lol hopefully stays fixed gonna stress test the crap outta of it seems if it holds up

A zepher... I just had a very similar issue. Check the secondary error code, Im sure your going to get 0013. If it is that then check this thread out here https://www.acidmods.com/forum/index.php/topic,41541.0.html

With the xbox I was having trouble with, it said cpu reworked on the motherboard. I think what happened was (assuming you have the secondary 0013 code) that something is up with the ram which caused your cpu to get very hot and you got a cold solder joint underneith. Got 3RL or whatever send it to microsoft. They reflowed the cpu, it turned on afterwards, so they shut it off and sent it back. Of course they are idiots and would not leave it on to see how long it would last so they know nothing of the overheating issues. This is all just speculation though.
